OBJECTIVE The absence of cerebral autoregulation in preterm infants has been associated with adverse outcome, but its bedside assessment in the immature brain is problematic. We used spatially resolved spectroscopy to continuously measure cerebral oxygen saturation (expressed as a tissue-oxygenation index) and used the correlation of tissue-oxygenation index with spontaneous fluctuations in mea...

This prospective observational study on 36 neonates aimed to estimate the correlation between the new Saturation Oxygen distending Pressure Index (SOPI) and Oxygenation index. SOPI had high correlation (r=0.94) with oxygenation index. SOPI of <2, 2, and 3.7 represented mild, moderate and severe pulmonary disease, respectively with high sensitivity and specificity.

INTRODUCTION Absence of pulmonary perfusion during cardiopulmonary bypass (CPB) may be associated with reduced postoperative oxygenation. Effects of active pulmonary artery perfusion were explored in patients with chronic obstructive pulmonary disease (COPD) undergoing cardiac surgery. METHODS 90 patients were randomised to receive pulmonary artery perfusion during CPB with either oxygenated ...

Peripheral tissue oxygenation has been studied with near-infrared spectroscopy (NIRS) on either the forearm or calf with questionable comparability. The aim was to compare forearm and calf tissue oxygenation in healthy term neonates measured with NIRS. Fractional oxygen extraction, tissue oxygenation index, and mixed venous oxygenation were similar in both extremities, whereas oxygen delivery a...

OBJECTIVE Although all definitions of acute respiratory distress syndrome use some measure of hypoxemia, neither the Berlin definition nor recently proposed pediatric-specific definitions proposed by the Pediatric Acute Lung Injury Consensus Conference utilizing oxygenation index specify which PaO2/FIO2 or oxygenation index best categorizes lung injury. We aimed to identify variables associated...

Inhaled nitric oxide (iNO) improves oxygenation in term and near-term infants with persistent pulmonary hypertension of the newborn (PPHN) and decreases the need for treatment with extracorporeal membrane oxygenation (ECMO). This mode of treatment is currently being introduced in Malaysia. We report our preliminary experience using low dose inhaled nitric oxide (20 parts per million) in three n...

In this issue of Pediatrics, Fyfe et al present convincing evidence that cerebral tissue oxygenation index (TOI) is lower in habitually supinesleeping preterm infants placed prone rather than supine, and that the cerebral TOI for preterm infants placed prone is lower than the TOI for term infants placed prone. Increased rates of sudden infant death syndrome (SIDS) among all infants sleeping pro...
